- A link will be sent to this email that will allow you to create a password and then sign on to the portal.
- In the portal, please fill out the clinical profile and your medication history for our team
We will be using this portal for all communications – your questions, your documents, your receipts and your lab requisitions. You will also upload your outside labs into your portal.
