- Since this is your first time logging in, please click on the ‘New Sign Up’ button to set your initial password.
- A link will be sent to this email that will allow you to create a password and then sign on to the portal.
- In the portal, please fill out the clinical profile and complete the Demographic Details, Past Clinical History, and Cancellation Policy.
We will be using this portal for all communications – your questions, your documents, your receipts and your lab requisitions. You will also upload your outside labs into your portal.
